Case Management Registered Nurse (RN)

Job Overview

We are seeking a dedicated and experienced Case Management Registered Nurse (RN) to join our team. This position primarily focuses on discharge planning and requires a strong background in acute care case management.

Responsibilities

• Conduct comprehensive assessments and develop individualized discharge plans for patients.
• Collaborate with interdisciplinary teams to ensure continuity of care for patients post-discharge.
• Facilitate the appropriate placement of patients by coordinating resources and services.
• Monitor patient progress and adjust discharge plans as necessary to meet patients' needs.
• Provide education and support to patients and families regarding discharge plans and post-care needs.

Qualifications

• Current nursing license (RN) in good standing.
• A minimum of 2 years of case management experience, preferably in an acute care setting.
• At least 6 months of experience specifically in discharge planning within a large organization or teaching hospital.
• Strong communication and interpersonal skills.
• Ability to work collaboratively in a team-oriented environment.

Schedule

The position offers a full-time schedule of 40 hours per week, with day shift availability from Monday to Friday. Work hours may vary based on operational needs, with options for either four 10-hour shifts or five 8-hour shifts.
