RN - Case Management

What You’ll Do:

• Lead infection control efforts by developing and updating policies, conducting evaluations, and educating staff on safety procedures and new equipment.
• Manage employee health programs, maintain accurate employee health profiles, and ensure compliance with infection prevention standards.
• Provide expert case management to patients with complex healthcare needs, coordinating and assessing outpatient referrals for medical necessity and cost-effectiveness.
• Collaborate with healthcare providers, staff, and external facilities to align care plans with regulatory standards and quality improvement goals.
• Monitor statistical data related to patient consultations and referrals, supporting continuous quality improvement initiatives.

Required Skills:

• Active RN license with a minimum of 3 years of clinical experience, specifically with infection control expertise.
• Strong knowledge of infection prevention protocols, safety guidelines, and employee health management.
• Effective care coordination and case management skills, with the ability to review and monitor outpatient services.
• Excellent communication and collaboration abilities to work effectively with multidisciplinary teams and external partners.

Nice to Have Skills:

• Prior experience working within federal or public health settings.
• Background in public health or community health programs.
• Familiarity with CMS criteria and healthcare compliance standards.

Preferred Education and Experience:

• Registered Nurse (RN) licensure in good standing.
• Minimum of 3 years of clinical experience, with demonstrated infection control expertise and case management background.
