Active California RN License
BLS — AHA or ARC accepted
Nursys verification uploaded within the last 2 weeks
Required Experience
Minimum 1 year RN experience
Must have Home Health Case Manager experience — this is specifically required
Experience with Home Health
Experience with care coordination
Experience with discharge planning
Experience with disease management
Previous Charge experience required
Strong experience managing a patient caseload and coordinating interdisciplinary care
Preferred
Epic experience
Experience working with geriatric patients
Experience coordinating with PT, social services, transportation, and other interdisciplinary teams
