Position Overview:

The RN – Purchased/Referred Care (PRC) Case Manager will support the Fort Peck Service Unit by coordinating and managing healthcare services for eligible patients requiring care through the Purchased/Referred Care program.

The RN will work closely with patients, healthcare providers, facilities, and other members of the care team to facilitate appropriate referrals, coordinate services, monitor patient needs, and support continuity of care.

Key Responsibilities

• Coordinate and manage patient care and referrals through the Purchased/Referred Care (PRC) program.
• Review patient needs and assist with appropriate healthcare referrals and services.
• Coordinate with healthcare providers, specialty facilities, and other healthcare organizations.
• Monitor referrals and patient care to support continuity and timely access to services.
• Maintain accurate and timely patient records, case documentation, and referral information.
• Communicate with patients and healthcare providers regarding appointments, referrals, and care coordination.
• Assist with resolving issues related to referrals and access to healthcare services.
• Follow applicable facility, program, and regulatory policies and procedures.
• Maintain confidentiality of patient information and follow applicable privacy requirements.
• Perform other related nursing and case-management duties as assigned.

Requirements:

• Current and unrestricted Registered Nurse (RN) license.
• Relevant nursing and/or case management experience preferred.
• Strong communication, organization, and care-coordination skills.
• Ability to work independently and collaboratively with healthcare providers and multidisciplinary teams.
• Must successfully complete all required facility and agency compliance, background checks, and clearance requirements before starting.

Schedule:

8x5 – Monday through Friday

Assignment:

6 Months+

Start: As soon as cleared.
