RN – Case Manager 

Position: Registered Nurse – Case Manager
Location: San Ramon, California
Contract: Standard Travel Contract
Schedule: 5×8-hour shifts
Shift:8:00 AM – 4:00 PM (08:00–16:30)
Guaranteed Hours: 40 hours/week
Experience Required:2 years

The RN Case Manager is responsible for coordinating and managing patient care across the healthcare continuum. The role involves assessing patient needs, developing care plans, coordinating services, and collaborating with providers, patients, and multidisciplinary teams to support quality and cost-effective outcomes.

• Perform comprehensive patient assessments and identify care needs.
• Develop, implement, and monitor individualized care plans.
• Coordinate care with physicians, healthcare providers, and multidisciplinary teams.
• Facilitate patient transitions, admissions, discharges, and follow-up care.
• Monitor patient progress and identify barriers to effective care.
• Provide patient and family education regarding treatment plans and available resources.
• Coordinate referrals, services, equipment, and community resources as needed.
• Maintain accurate and timely documentation in the EMR.
• Advocate for patients and help ensure continuity of care.
• Monitor utilization and support appropriate healthcare resource management.

• Active RN license
• Minimum 2 years of Case Management experience
• Strong care coordination and clinical assessment skills
• Excellent communication and organizational skills
• Experience working with multidisciplinary healthcare teams
• EMR documentation experience

Schedule: Monday–Friday 
Guaranteed:40 hours/week
