Cerner experience required; Epic experience preferred
CCM or ACM certification preferred
ED experience is a strong asset

Responsibilities
Assess, plan, implement, monitor, and evaluate individualized patient care plans across the continuum of care.
Coordinate discharge planning with physicians, social workers, and multidisciplinary teams.
Ensure appropriate utilization of healthcare resources while maintaining quality patient outcomes.
Review medical necessity using MCG guidelines and regulatory standards.
Collaborate with inpatient units including Med-Surg, Med-Tele, Surgical, ICU, and ED as assigned.
Maintain accurate documentation and communicate effectively with patients, families, and healthcare providers.
