Job Title: Habilitation Nurse

Hours per Shift: 8:00 AM - 5:00 PM
Experience: Minimum 2 years of recent RN experience working with individuals in home health, hospice, long-term care, or case management.
License: Registered Nurse licensed to practice in the Client, active and in good standing without restrictions.
Certifications: Additional training or certification in behavioral health, complex medical conditions, or nursing case management is preferred.
Must-Have: Excellent communication skills, including the ability to explain complex information effectively. Proficient with Google Office, Microsoft Office, and Adobe Acrobat.
Description:
We are seeking the support of contract nurses to assist with rising nursing caseloads.
Locations will vary, and the position's duty post will be defined after the selection of candidates.
The role involves conducting nursing assessments and collaborating with external agencies to ensure appropriate services are delivered.
Assessments are used to authorize skilled nursing services for members.
The successful candidate will conduct in-person visits statewide and monitor health needs.
They will assist with referrals and advocate for members' rights.
The Registered Nurse will complete a report for each member documenting the medical necessity for skilled services.
In-person assessments will be required every 90 days and as needed.
The position is field-based, and travel is necessary to complete in-person assessments.
Responsibilities include completing nursing referrals, placing vendor calls, and completing documentation within the established timeframe.
Interprofessional collaboration both internally and externally is essential.
Participation in multidisciplinary planning meetings will help determine appropriate services.
Collaboration with members and their support systems is required.
Attendance at mandatory meetings and completion of required annual training will be expected.
