Job Title: Registered Nurse Profession: Nursing Specialty: Staff Augmentation Duration: 45 days Shift: Day Hours per Shift: 8:00 AM - 5:00 PM Experience: Minimum of 2 years of recent RN experience working with individuals in home health, hospice, long-term care, or case management. License: Registered Nurse licensed to practice in the Client, active and in good standing without restrictions. Certifications: Additional training or certification in behavioral health, complex medical conditions, or nursing case management is preferred. Must-Have: Excellent communication skills, including advanced abilities in explaining complex information to members and their support systems. Proficient with Google Office, Microsoft Office, and Adobe Acrobat, including instant messaging features and document uploading. Description: We are seeking contract nurses to support the rising nursing caseloads being experienced statewide. The role involves conducting nursing assessments and collaborating with external agencies to ensure appropriate services are delivered. Assessments are utilized to authorize skilled nursing services for members. The successful candidate will conduct in-person visits and monitor health needs, assist with referrals, and advocate for member rights. The Registered Nurse will complete a report for each member, documenting the medical necessity for skilled services. In-person assessments are required every 90 days and as needed. This is a field-based position, and travel is required for in-person assessments. Responsibilities also include completing nursing referrals, placing vendor calls, and completing special projects as needed. Timely documentation is essential in accordance with established timeframes. The role involves interprofessional collaboration internally and externally. Participation in multidisciplinary planning meetings is required to determine and identify appropriate services. Collaboration with members and their support systems is essential. Attendance at mandatory meetings and completion of required annual training is expected.
