Certifications: Additional training or certification in behavioral health, complex medical conditions, or nursing case management is preferred
Must-Have: Excellent communication skills, including advanced abilities in explaining complex information to members and their support systems. Proficient with Google Office, Microsoft Office, and Adobe Acrobat, including instant messaging features, document uploading, and willingness to learn new software for documentation.
Description:
We are seeking the support of contract nurses to assist with rising nursing caseloads.
Locations will vary and will be defined after candidates are selected.
The role involves conducting nursing assessments and collaborating with external agencies to ensure appropriate services are delivered.
The assessments authorize skilled nursing services for members.
Successful candidates will conduct in-person visits statewide and have a core district they serve.
As a Habilitation Nurse, you will monitor health needs, assist with referrals, and advocate for members' rights.
You will complete a report for each member, documenting the medical necessity for skilled services to meet individualized needs.
In-person assessments will be conducted every 90 days and as needed.
The position is field-based, requiring travel for in-person assessments.
You will complete nursing referrals and vendor calls as needed.
Responsibilities also include completing documentation in the timeframe established.
Interprofessional collaboration internally and externally is essential.
Participation in multidisciplinary planning meetings to identify appropriate services per established policies is required.
Collaboration with members and their support systems is essential.
Attendance at mandatory meetings and completion of required annual training is necessary.
