Job Title: Habilitation Nurse Profession: Registered Nurse Specialty: Staff Augmentation Duration: 45 Days Shift: Day Hours per Shift: 8:00 AM - 5:00 PM Experience: Minimum of 2 years recent RN experience working with individuals in home health, hospice, long-term care, or case management. License: Registered Nurse licensed to practice in the Client, active and in good standing without restrictions. Certifications: Additional training or certification in behavioral health, complex medical conditions, or nursing case management is preferred. Must-Have: Excellent communication skills, including advanced abilities in explaining complex information to members and their support systems. Proficient with Google Office, Microsoft Office, and Adobe Acrobat, with a willingness to learn new software used for documentation. Description: We are seeking the support of contract nurses to address the increasing nursing caseloads. Locations will vary, and the position's post of duty will be defined after selection. We are looking for contract nurses to support rising nursing caseloads. Five contract nurses will be needed to add to the existing District Nurses currently on staff. These nurses will support members with skilled nursing needs. Contract nurses will be utilized differently from current District Nurses. Each contract nurse will serve as a floater, stepping in for nurses on annual leave, sick leave, or leave of absence. Key functions will be in various districts. The role involves conducting nursing assessments and collaborating with external agencies to ensure appropriate services are delivered. Assessments will be used to authorize skilled nursing services for members. The successful candidate will conduct in-person visits statewide and monitor health needs. They will assist with referrals and advocate for members' rights. The Registered Nurse will complete reports documenting medical necessity for skilled services. In-person assessments will be required every 90 days and as needed. The position is field-based, requiring travel for in-person assessments. Responsibilities include completing nursing referrals and documenting in a timely manner. Collaboration with members and their support systems is essential. Attendance in mandatory meetings and completion of required annual training is necessary.
