The Case Manager is responsible for coordinating, planning, and facilitating services for individuals to ensure their healthcare, social, and/or support needs are met.
This role involves assessing client needs, developing care plans, monitoring progress, and collaborating with interdisciplinary teams to achieve optimal outcomes.
Key Responsibilities include:
Conducting comprehensive assessments of client needs, including medical, social, and financial factors.
