Job Description

Coordinate and manage patient care throughout the hospital stay.
Develop, implement, and monitor individualized case management plans.
Collaborate with physicians, patients, families, nurses, social workers, and interdisciplinary teams.
Evaluate patient needs and coordinate appropriate levels of care and healthcare resources.
Support utilization management and ensure cost-effective delivery of patient care.
Coordinate discharge planning and transitions to appropriate post-acute care services.
Communicate with insurance companies, payers, and review organizations regarding patient care and utilization.
Maintain clear, accurate, and timely case management documentation.
Educate physicians and staff regarding utilization, level-of-care, and discharge planning issues.
Identify barriers to discharge and coordinate appropriate solutions with the care team.
Maintain compliance with applicable federal, state, payer, and hospital regulations.
Participate in departmental meetings, quality improvement initiatives, and continuing education.
