Job Title: Case Manager RN Profession: Registered Nurse Specialty: Case Management Duration: 12 Weeks Shift: Day Hours per Shift: 8 Experience: Minimum of 1 year in a relevant specialty License: RN Licensure required Certifications: BLS required Must-Have: - Acute Care Experience - Discharge Planning/Care Coordination Experience - Strong preference for diverse Case Management experience, including Medical Surgical, Length of Stay, and Emergency Department Description: Registered Nurse or Master of Social Work needed for a Case Management position. The start date is as soon as possible. The schedule is 4x8 days, with a 72-hour guarantee per two-week period. Candidates must have experience in Acute Care settings and demonstrate proficiency in Discharge Planning and Care Coordination. Weekends are required every other week. Candidates will be floating between various units of the facility. Behavioral health experience does not count as Acute Care hospital experience. Candidates must possess charting experience in the EPIC system. All travelers are expected to float between like units based on skill sets. Candidates must submit a minimum of two supervisory references covering one year of employment within the last three years in a comparable specialty. Commitment to work three of the five designated holidays is required. All requirements and qualifications must be met for consideration. Completion of required modules is necessary prior to the start of the assignment.
