Job Title: Registered Nurse Profession: Nursing Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (07:30 - 15:00) Experience: Minimum of 1 year required. License: RN Licensure required. Certifications: BLS required. Must-Have: - Acute Care Experience - Discharge Planning/Care Coordination Experience - Diverse Case Management experience, including Med Surg and LLOS Description: This position involves working in a unit focused on care coordination and discharge planning within an acute care facility. The candidate will work in various areas of the facility, including different campuses. Weekends are required every other weekend. First-time travelers are accepted. Candidates with relevant acute care experience are preferred. Travelers are expected to float between like units. Behavioral health experience does not count as acute care hospital experience. References from supervisory roles are required, covering at least one year of employment in a similar specialty. Completion of required training and modules is expected prior to starting the position. All candidates must commit to working a minimum number of holidays throughout the contract. The number of patient ratios and unit-specific details will be provided during the hiring process. Strong communication skills are necessary for successful care coordination. The ability to adapt to various patient populations, including neonates, adolescents, adults, and geriatrics, is essential.
