The overall goal of the position is to enhance the quality of patient management and satisfaction. It aims to promote continuity of care and cost effectiveness through the integration of case management, utilization review, and discharge planning. The Care Manager must be a highly organized professional with great attention to detail. This role requires adaptability to frequent changes and compliance with regulatory and departmental guidelines and policies. Key Responsibilities: Identify cases and prioritize daily tasks. Review work lists to prioritize patients and identify new admissions. Conduct and document assessments and plans of care as per departmental guidelines. Participate in daily care management touchpoints according to established protocols. Communicate with the care management team to share priorities when necessary. Attend and actively participate in meetings to provide and receive updates on patient progression. Alert the care team to concerns impacting anticipated discharge of the patient. Modify discharge plans based on information shared in team meetings. Assist with the identification of expected discharge dates. Complete follow-up tasks from meetings as appropriate. Attend weekly complex care meetings. Present on patients during these meetings and collaborate to problem-solve issues. Proactively identify high-risk cases that require escalation. Discuss barriers to discharge and psychosocial concerns within a multidisciplinary team. Coordinate family meetings as needed to support the progression of care. Provide education on community resources and appropriate support groups. Educate and coordinate referrals to community resources and post-acute providers. Communicate medical milestones for transition to patients and families. Identify patients with discharge barriers based on ongoing assessments. Monitor patients throughout the day to ensure appropriate care progression. Assess discharge plans to determine post-discharge needs and communicate accordingly. Identify required authorizations for post-discharge services. Participate in medication resource management for non-resourced patients. Verify patients' understanding and agreement of discharge plans. Refer administrative tasks to appropriate support staff. Consult relevant healthcare professionals as mandated by established protocol.
