Seven Healthcare is seeking an experienced and highly organized Travel Registered Nurse (RN) - Home Health Case Manager for a contract opportunity in Pleasant View, TN offering $2,100 weekly. This 5x8s day shift, you will Will be covering Montgomery/Cheatham/Robertson County and Sumner area as needed, aswell as a mileage reimbursement of $0.41 per mile.

Key Responsibilities of the Travel Home Health Case Manager:

Coordinate patient care plans and transitions from admission through discharge
Perform utilization review and ensure medical necessity for hospital services
Collaborate with interdisciplinary teams, including social workers and providers
Educate patients and families on resources, follow-up care, and community support
Advocate for optimal health outcomes and efficient use of healthcare resources
Maintain accurate documentation and compliance with regulatory standards

Requirements of the Travel Home Health Case Manager:

Active RN license in Tennessee (or compact eligible)
BLS certification required
Vaild Drivers licence and Vaild Car Insurance required
2+ years of recent experience in case management
Strong knowledge of discharge planning, insurance authorization, and community resources
Strong experience in Hospice and HCHB.
