Engage with patients and their families to provide education and support. Participate in quality improvement initiatives within the facility. 2 years of acute (hospital) Case Management experience. Knowledgeable in Meditech, Midas, and Interqual. Must be available to work every other weekend. Strong communication and interpersonal skills.
