Description:
The role involves managing patient care in a case management setting.
Responsibilities include coordinating discharge planning for patients.
You will work with a multidisciplinary team to ensure optimal patient outcomes.
The patient-to-nurse ratio is variable, with a typical maximum of 25 patients.
Availability of unit secretaries and techs will support nursing staff.
You will utilize various documentation systems in your daily tasks.
Workdays are Monday through Friday, with rotating weekend shifts required.
On-call duties are not applicable for this position.
Floating to other units may be necessary based on staffing needs.
Maintaining a high standard of patient experience is a priority.
The position requires adaptability and teamwork to meet patient needs.
