Schedule: Alternating weekends with holiday rotation in accordance with facility policy

Requirements:

• Active Florida Registered Nurse license or compact state license
• Minimum 1-2 years acute care Case Management experience required
• Experience with Meditech and Midas EMR systems strongly preferred
• Strong knowledge of discharge planning, utilization review, and care coordination
• Excellent communication and interdisciplinary collaboration skills
