Key Responsibilities of the RN - Case Management- Coordinate and manage patient care plans effectively
- Conduct complex discharge planning
- Utilize Midas and Meditech systems for patient documentation
- Work collaboratively with healthcare teams, including RN CM and SW CM
- Maintain a patient ratio of at least 25:1 CM
Requirements of the RN - Case Management- Valid RN license
- Experience with Meditech and Midas systems
- Proven experience in case management and discharge planning
