Job Title: Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 8 hours Experience: Minimum Years of Experience: 2 years License: Pending license accepted but must have license prior to start date. Certifications: BLS required. Must-Have: Experience as complex case manager, preferably with NCQA experience. Experience in care coordination and discharge planning. Utilization of InterQual criteria and Milliman guidelines. Description: This position is for a registered nurse specializing in case management. The case manager will work with chronic diagnoses including high-risk pregnancy, postpartum, diabetes, congestive heart failure, chronic obstructive pulmonary disease, asthma, obesity, and behavioral health. The role involves making calls to enrollees in the designated area to complete comprehensive assessments. The case manager will develop care plans and manage ongoing care within the electronic medical record system. Care plans must be individualized and address social determinants of health needs. Collaboration with hospitals, physicians, and other stakeholders is essential for successful execution of care plans. Documentation must comply with required standards and timelines. The case manager must be comfortable providing care to a diverse age range, from newborns to geriatric populations. Ability to operate a computer and utilize communication platforms is necessary. Previous charge experience is preferred. Knowledge of regulatory requirements and accreditation standards is required. This position does not require on-call responsibilities or weekend shifts.
