Travel Nurse RN - Case Manager - $2,274 per week in New York City, NY

Job Title: Care Manager Profession: Care Manager Specialty: ED Duration: 8 months Shift: Full time Hours per Shift: 35 hours per week (9 am - 5 pm, Monday to Friday) Experience: Minimum of five years of clinical experience as a Registered Nurse required. Minimum of two years of prior experience in a health care setting, Care Management, or Managed Care setting required. Leadership experience preferred. License: RN Licensed as a Registered Nurse in the State of New York required. Certifications: Baccalaureate degree required. PRI and Screen certification within three months of employment is required. Master’s Degree and Bilingual Spanish Certification in Case Management preferred. Must-Have: - Healthcare Management - RN License - Managed Care Description: The primary goal of the Care Manager is to optimize members' health care and delivery of care experience. Expected cost savings are due to improved quality of care. This is accomplished through engagement and understanding of the member's needs, environment, providers, support system, and optimization of services available to them. The Care Manager is expected to assess and evaluate the member's needs. The Care Manager must be a creative, efficient, and resourceful problem solver. In collaboration with the member's care team, a plan of care with individualized goals and interventions is developed, implemented, and evaluated for outcomes. Computer literacy is required. The ability to proficiently read and interpret medical records, claims data, pharmacy records, lab reports, and prescriptions is required. The ability to work closely with the member and caregiver is essential.
