TravelNurseSource is working with Cynet Health to find a qualified Case Manager RN in New York City, New York, 10004!

Job Title: Care Manager Profession: Care Manager Specialty: ED Duration: Full Time Shift: Monday, Tuesday, Wednesday, Thursday, Friday Hours per Shift: 9 AM - 5 PM Experience: Minimum of five years of clinical experience as a Registered Nurse. Minimum of two years of prior experience in a health care setting, Care Management, or Managed Care setting required. License: RN Licensed as a Registered Nurse in the State of New York required. Certifications: Baccalaureate degree required. PRI and Screen certification within three months of employment is required. Master’s Degree and Bilingual Spanish Certification in Case Management preferred. Must-Have: - Healthcare Management - RN License - Managed Care Description: The primary goal of the Care Manager is to optimize members' health care and enhance the delivery of care experience. This will lead to expected cost savings due to improved quality of care. The role involves engagement and understanding of the member's needs, environment, providers, support system, and optimization of available services. The Care Manager is expected to assess and evaluate member's needs while being a creative, efficient, and resourceful problem solver. In collaboration with the member's care team, a plan of care with individualized goals and interventions will be developed, implemented, and outcomes evaluated. Computer literacy is required. Ability to proficiently read and interpret medical records, claims data, pharmacy, lab reports, and prescriptions is necessary. Leadership experience is preferred.
