Description:
RN Case Manager for Home Health, Full Time, Days.
This role allows you to develop meaningful relationships with patients and become an integral part of their care team.
You will have a broad independence while working cohesively within a patient-care team.

Responsibilities include:
Screening, evaluating, re-evaluating, and treating patients.
Coordinating and communicating treatment activities with interdisciplinary team members and the patient.
Developing care plans in collaboration with the patient's physician, the patient, and their family or primary caregivers upon admission.
Periodically reviewing and recertifying the patient care plan as appropriate.
Coordinating the patient care plan through interdisciplinary communication and case conferences when needed.
Advising other disciplines of discharges in a timely manner.
Providing ongoing assessments of the patient's clinical, functional, and emotional status to identify problems and progress toward desired goals.
Communicating significant changes to the patient's physician and relevant colleagues.
Documenting evaluations, treatment goals, and objectives.
Teaching the patient and caregiver self-care techniques as appropriate.
Providing medication, diet, and other instructions as ordered by the physician and utilizing opportunities for health counseling with patients and caregivers.
