Job Title: Hospice Nurse Profession: RN Specialty: Hospice RN Duration: 13 weeks Shift: Days Hours per Shift: 8:30am - 5:00pm (Schedule can be flexible based on patient's availability.) Experience: Minimum of 1 year of Hospice experience License: Active RN license Certifications: BLS certification required Must-Have: One year of Hospice experience Description: The Hospice Nurse admits patients to service. They assess the patient's and family's physical, psychosocial, environmental, safety, and developmental needs. The nurse develops an individualized plan of care based on their assessment. They document their observations and interventions related to the patient's environment. This role coordinates care with a multidisciplinary team. They participate in the hospice program's quality assessment performance improvement program. The nurse conducts follow-up visits as needed. Territory details include various cities and neighboring areas as necessary. Holiday coverage is required.
