Job Title: Hospice Registered Nurse

Hours per Shift: 9:30 AM - 6:00 PM (Schedule can be flexible based on patient's availability.) One weekend required.
Experience: Minimum of 1 year of hospice experience
License: Valid RN license required

Description:
The Hospice Nurse admits patients to service.
Assesses the patient's and family's physical, psychosocial, environmental, safety, and developmental needs.
Develops an individualized plan of care based on assessments.
Documents observations and interventions related to the patient's environment.
Coordinates care with a multidisciplinary team.
Participates in the quality assessment performance improvement program.
Conducts follow-up visits as needed.
Territory covers Millbrae, Burlingame, San Mateo, Foster City, Belmont, Redwood City, and neighboring cities as needed.
Holiday coverage is required.
