Description:
Looking for a Registered Nurse or Master of Social Work with a focus on Case Management.
This position requires strong experience in acute care settings and discharge planning.
Candidates must be willing to work weekends, alternating as necessary.
Experience across various medical-surgical units is preferred.

The ideal candidate will have experience with care coordination and the ability to float between different units as needed.
Charting is done using the EPIC system.
Strong interpersonal skills and the ability to work in a fast-paced environment are essential.
This role interacts with a wide range of patient age groups, including neonates, adolescents, adults, and geriatrics.
Behavioral health experience will not be considered as part of acute care hospital experience.
All candidates must provide two supervisory references covering one year of employment in a comparable specialty setting.
Travelers are expected to float between various units as required.
Candidates must commit to working three holidays out of five designated holidays during the contract term.
