Job Title: Case Manager Registered Nurse Profession: Registered Nurse Specialty: Case Management Duration: 8 Weeks Shift: Day 4x10-Hour Hours per Shift: 10 Experience: 1 year License: RN Licensure Certifications: BLS Must-Have: Acute Care Experience, Discharge Planning/Care Coordination Experience Description: RN or MSW needed for a 4x10 days contract. Must have at least 1 year of experience in care coordination and discharge planning. Candidates with diverse case management experience are preferred, including Medical Surgical experience. Weekends are required - every other weekend. First-timers are accepted; however, acute care experience is mandatory. Candidates may be required to float between various areas within the facility, including all departments. Experience in a fast-paced environment is essential. Behavioral health experience does not count toward acute care experience. Travelers are expected to float between like units within their skill set. Strong preference will be given to candidates with discharge planning and care coordination experience. Candidates must commit to working 3 of the 5 major holidays during the contract period. References are required from the last 3 years covering at least 1 year of experience. Providers may need to use communication devices as needed for the position. Modules are required to be completed pre-start and annually.
