Special Procedures/Unit Details : RN Care Manager assists patients and families to develop and fulfill a safe discharge plan to leave the hospital with the resources available to the individual patient. RN CM assists with durable medical equipment, home health, hospice, shelters and multiple types of post-acute placements such as Skilled Nursing Facilities, adult foster homes, and assisted living. works with multiple community partners to arrange plans and secure authorization from insurance companies. This role does have Case Management Assistants who help with various tasks such as: SNF auth, transport, DME dispensing, calling pharmacy, 2nd IMM delivery, placement referrals, Medicaid long term care referrals and more. This role does not provide utilization review, but knowledge or UR helpful.Weekend coverage focuses on discharges and new assessments only, no patient progression or full assignment of cases, the team divides by units and assists each other as able. Special Equipment : - #Tier3 Travel Compliance SSN DOB RTO - 6 days total or 3 consecutive days can be auto-approved . Anything more, traveler will sign contract with pending RTO. *Anything more than 6 Days will potentially delay your offer while we run approval by the client. To minimize turn around please limit total RTO requests. Must commit to working 3 of the 5 holidays (Thanksgiving, Xmas Eve, Xmas Day, NYE, NYD) MUST request all RTO / specific scheduling requests up-front during submittal.
