No first-time travelers are eligible.
Candidates must be located outside a 50-mile radius from the facility.
Description:
The purpose of this position is to provide ongoing support and expertise through comprehensive assessment, planning, implementation, and evaluation of individual patient needs.
The overall goal is to enhance the quality of patient management and satisfaction.
This includes promoting continuity of care and cost-effectiveness through integrated functions of case management, utilization review, and discharge planning.
The Case Manager must be a highly organized professional with great attention to detail.
The individual must be adaptable to frequent change and compliant with regulatory and departmental guidelines and policies.
