Description: Chesapeake is located in a beautiful coastal region, perfect for outdoor lovers.
The position involves care management for adult patients.
The schedule consists of 5x8 days, working weekdays, with some weekends and holidays.
Travelers are expected to work two of the three winter holidays.
Candidates must have recent experience in an adult acute care hospital setting with discharge planning.
They should have less than a 6-month gap since their last acute care Case Management role.
Experience in a Level I, II, or III acute care hospital is necessary.
Candidates will manage a patient census of approximately 15 to 25 patients.
They should be rated 4/5 or 5/5 across all Case Management functions, including knowledge of CMS regulations.
Epic proficiency is mandatory.
Previous travel experience is strongly preferred, though first-time travelers with extensive acute care Case Management experience may be considered.
Experience in acute care discharge planning and Utilization Review is required.
Candidates must be able to effectively manage a high-volume, fast-paced patient population.
The facility will not consider candidates with any license or background issues.
Candidates are encouraged to review their history before applying.
The position provides interdisciplinary support and involves various patient diagnoses.
It requires managing appeals and denials, audits, and case coordination.
Compliance with regulations from relevant health authorities is expected.
