Description:
The RN Care Manager will oversee a daily caseload of patients on the inpatient units with a variety of care needs.
The RN will attend interdisciplinary rounds.
The RN will complete initial assessments.
The RN will facilitate the development of a plan of care in collaboration with the patient, family, caregiver, and health care team.
The RN will communicate consistently with medical teams and patients regarding progress of care needs related to post-discharge care needs.
This includes home health and inpatient rehabilitation.
The RN will interact with post-acute care referral sources to establish a plan.
The RN will document and place orders for home health, durable medical equipment, and nutritional needs within the EMR system.
