Job Summary:We are seeking an experienced RN Case Manager / Patient Care Coordinator to oversee and coordinate care for acute inpatient populations. The role focuses on discharge planning, care coordination, and ensuring safe, cost-effective patient outcomes.
The candidate will collaborate closely with physicians, nursing staff, patients, families, and interdisciplinary teams to facilitate smooth transitions of care and prevent hospital readmissions.
Key Responsibilities:- Coordinate and manage patient care throughout the inpatient stay
- Develop and implement discharge plans in collaboration with patients, families, and healthcare teams
- Work with physicians and ancillary staff to ensure appropriate care progression
- Facilitate transitions to community providers and outpatient care
- Collaborate with utilization review and case management teams
- Ensure compliance with clinical and cost-effective care standards
- Support initiatives to reduce readmissions
Required Qualifications:- Active RN License (State License Required)
- BLS Certification
- Experience in case management, discharge planning, or care coordination
- Strong communication and interpersonal skills
- Ability to work in a fast-paced acute care environment
