Shift Type: Monday-Friday, 7:30 AM - 4:00 PM (Day), 5x8 with every other weekend required

Hours per Week: 40

Candidate Pay Rate: $68 per hour

Roles & Responsibilities
  • Coordinate safe, timely discharges by creating and managing individualized plans of care
  • Perform needs assessments and arrange DME and community resources to support transitions
  • Collaborate with interdisciplinary teams (nursing, social services, physicians) to optimize patient flow
  • Use Epic for thorough, timely charting and care documentation
  • Perform utilization review and case management activities in line with CMS/regulatory expectations
  • Float across hospital areas (including Colby and Pacific campuses) to meet patient needs
  • Support acute care units including Med Surg, ED, and ICU with discharge planning expertise
  • Champion patient-centered care and clear communication with patients, families, and post-acute partners
