Job Title: Case Manager RN Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 8 hours Experience: Minimum of 2 years of acute care experience required License: Valid RN license (pending license acceptable) Certifications: BLS Must-Have: - Experience with discharge planning and care coordination - Experience with real-time documentation of assessments and notes - Familiarity with MCG (formerly Milliman Care Guidelines) Description: Case Manager RN needs to work 5x8 hour shifts from 08:00 to 16:30 on weekdays. The clinician will cover 35-40 patients per day. This role is not focused on utilization review or inpatient authorization, but may involve requesting authorizations for post-acute placement or discharge medications. Real-time documentation during assessment is required. Experience with risk assessments and social determinants of health (SDOH) is a plus. The dress code is business casual, and a white lab coat or jacket is required when out on the floor. The department supports interdisciplinary collaboration with physical therapy, social services, and other support services. Patient demographics include adolescents, adults, pediatrics, and geriatrics. This position may require floating within the scope to meet facility needs.
