Job Title: Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 8 Experience: - Minimum of 2 years of Acute Care Case Management/Discharge Planning experience required. - Client experience is a must. - Behavioral health experience does not apply. License: - California state license required. - Pending license accepted, but must be licensed upon start date. Certifications: - Relevant certifications as a Registered Nurse. Must-Have: - Experience with assessing patients. - Proficient in care coordination, discharge planning, and utilization review. - Competent in arranging durable medical equipment (DME), skilled nursing facilities (SNF), and ambulance transport. Description: This position requires a seasoned traveler, no first-time travelers. The ratio for daytime shifts is 1 nurse to 25 patients. Weekend shifts require a nurse to be available for a ratio of 1 to 45 patients. Knowledge of adoption/foster care, drug and alcohol abuse treatment options, and mental health issues is essential. Experience with health maintenance organizations (HMOs) and independent practice associations (IPAs) is preferred. Ability to organize and participate in family meetings is necessary. Participation in the development of treatment plans is expected. The position involves working with a diverse patient population, including neonates, adolescents, adults, and geriatrics. Case loads may reach up to 25 patients daily. Interdisciplinary support includes physical therapy and social services. Must demonstrate skills in care coordination and discharge planning. Completion of additional training modules may be required. Flexibility is required for covering weekend shifts as needed. Traveling nurses are expected to float between facilities within the state. Refusal to float may result in termination of the assignment. All selected candidates must provide references covering experience within the last three years.
