Job Title: Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 8 Experience: Minimum 2 years of acute care experience License: Current nursing license Certifications: Must have relevant certifications Must-Have: - Experience with HMOs, IPAs, advanced directives, and protective services processes - Current case management experience within the last year Description: Weekend requirements include working every other weekend. Acute care experience is necessary. First-timers are accepted as a strong candidate with clinical approval. Call is not required. Travelers may need to float between similar units based on need. Local candidates are also accepted. All requests for time off should be included at the time of submission. Experience with charting system, specifically required charting system experience is necessary. Daily case load may reach up to 25 patients. This role involves care coordination, discharge planning, and compliance with CMS regulations. Candidates should be familiar with HIPAA guidelines and DRG processes. Interdisciplinary support will be available, including social services. Travelers can expect to float between facilities within the assigned state. Failure to float may result in termination of assignment. Providers may be required to use communication devices needed to perform duties. Modules are non-billable, and completed pre-start and annually. Time spent on modules will be self-reported and recorded. References covering one year of experience within the last three years are required for compliance. All candidates must comply with holiday work requirements.
