Job Title: Case Manager Profession: Registered Nurse Specialty: Case Manager Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 16:30) Experience: Minimum 2 years of current acute hospital experience License: CA state license required; pending license accepted Certifications: Not specified Must-Have: Knowledge of Interqual and Epic Experience with care coordination and discharge planning Ability to assess patients and develop plans of care Description: The position requires a seasoned Case Manager with substantial experience in an acute care setting. Candidates must possess current knowledge of Interqual and Epic systems. The patient-to-case manager ratio is 1:20. Candidates should demonstrate effective care coordination abilities and discharge planning experience. The role includes conducting needs assessments and facilitating prior authorizations. Experience with HMOs and IPAs is preferred. Successful candidates must be flexible and able to work some weekends, with a minimum of 2 weekend shifts required in a 4-week schedule. The workload may involve managing a case load of approximately 20-22 patients. All travelers are expected to be adaptable and float between various facilities as needed. The position does not require specific certifications beyond nursing licensure. References covering a minimum of one year of experience are required during the compliance process.
