TravelNurseSource is working with Cynet Health to find a qualified Case Manager RN in Burbank, California, 91505!

Job Title: Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 16:30) Experience: Minimum of 2 years in acute medical care management, including care coordination and discharge planning. License: California state license required; pending license accepted but must be licensed upon start date. Certifications: Required experience with computerized documentation. Must-Have: - Experience with assessing patients, care coordination, and discharge planning. - Familiarity with Medicare and Medicaid guidelines, disease management, and HIPAA regulations. - Ability to manage a caseload of up to 25 patients. Description: The role involves providing care coordination and discharge planning for patients. Candidates must have a minimum of two years of experience in acute medical care management. The position does not require floating between different facilities. Weekend requirement typically involves working one full weekend every four weeks, based on departmental needs. Travel experience is required. Candidates should possess strong skills in assessing patients, preparing plans of care, and determining medical necessity. Familiarity with hospital systems and the ability to comply with documentation standards is necessary. This position involves working with diverse patient populations, including adults and geriatrics. Candidates must be prepared to commit to working three of the five major holidays.
