Job Title: Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 13 Weeks Shift: Night 5x8-Hour Hours per Shift: 8 Experience: 2 Years in Acute Care License: Active RN License Required Certifications: Basic Life Support (BLS) Preferred, Case Management Certification Preferred Must-Have: Acute care experience required. Experience with HMOs, IPAs, and advance directives. Knowledge of protective services processes. Description: Required to work every other weekend. First-timers accepted with clinical approval. Call not required. Traveler may need to float between units based on need. Local candidates are accepted. All requests for time off must be submitted during application. Experience with charting system, Epic, is required. Must have knowledge of CMS guidelines and HIPAA. Experience with discharge planning and benefits eligibility is essential. Will work with diverse patient age groups, including adolescents, adults, pediatrics, and geriatrics. Daily caseload may include up to 25 patients. Interdisciplinary support available, including social services. Experience in acute hospital case management must be current within the last year. References from the last three years are required, covering a total of one year. All travelers are expected to float between facilities within a given state. Failure to float may result in termination of the assignment. Communication devices will be provided as needed for job duties. Modules are non-billable and must be completed pre-start and annually. Time spent on modules is self-reported.
