Description
Job Title: Case Manager

Certifications: Must have relevant nursing certifications
Must-Have:
- Acute care experience
- Experience with HMOs, IPAs, etc.
- Experience with advance directives, POLST, protective services process
Description:
This position requires every other weekend work.
Candidates with at least 2 years of case management experience are encouraged to apply.
First-time travelers are accepted with clinical approval.
Call duty is not required for this position.
Travelers may need to float between similar units based on departmental needs.
Local candidates are welcome to apply.
All requested time off must be included at the time of submission.
Charting will be performed using a specific electronic health record system.
The facility provides assistance for travelers in finding housing if needed.
Candidates should be prepared to work in a setting that accommodates various patient age groups, including adolescents, adults, pediatrics, and geriatrics.
Daily caseload may include up to 25 patients.
Experience with admission criteria, appeals and denials, and care coordination is essential.
Knowledge of CMS regulations and discharge planning is required.
Strong understanding of HIPAA guidelines and national safety goals is necessary.
References from previous positions covering at least one year of experience are required.
Travelers are expected to float between facilities within the same state as needed.
Failure to comply with floating requirements may result in assignment termination or loss of guaranteed hours.
