The case manager utilizes a multidisciplinary team approach to negotiate, procure, coordinate, and monitor services and resources needed by patients. The case manager is responsible for reviewing the patient at admission and throughout the hospital stay for the appropriate level of care and utilization of resources. The case manager will perform transition care planning activities to secure appropriate post-hospital care arrangements. They will monitor the patient during the course of their hospitalization. The case manager participates in performance improvement initiatives. Organizational skills are essential. Proven ability to analyze and present data is required. Computer literacy is necessary, with data management skills preferred. Excellent interpersonal and communication skills are a must. Ability to maintain confidentiality is crucial. Collaboration and negotiation skills with providers, patients, families, insurance organizations, and government agencies are necessary. A proven ability to work independently is required. Excellent organizational skills are essential. Demonstrated ability to establish and maintain relationships with healthcare providers is necessary. Knowledge of local community resources is required. Knowledge of regulatory guidelines and reimbursement processes is essential.
