Job Title: Registered Nurse Profession: Nursing Specialty: Case Manager Duration: 13 weeks Shift: Day Hours per Shift: 8 Experience: Minimum 2 years License: Pending state license accepted Certifications: Required Must-Have: Experience with geriatric population highly preferred Description: The Registered Nurse Case Manager establishes a well-rounded care practice that includes patient assessment, care planning, implementation, coordination, and monitoring. Ongoing routine evaluation ensures medically and financially appropriate care. Responsibilities include facilitating interdisciplinary team care coordination and intensive medical management of participants recently discharged from acute care centers and skilled nursing facilities. This position involves working closely with the interdisciplinary team, including social workers and physicians, to collect necessary medical and social information. Cohesive decisions must be made to ensure the safety and well-being of participants. The role is primarily on-site, but there may be times when travel to a participant's residence for an assessment is needed. Mileage will be reimbursed for this time. The Case Manager will start from the center and leave to the community for visits. This role strives to keep older adults as healthy as possible while living in the community through clinics, home visits, and more. All Case Managers are assigned a laptop and cell phone to perform their duties. A system is used where case managers can create expense reports and track miles. Previous charge experience is preferred. The daily census includes geriatric patients with a patient ratio of 19. The number of visits per day is approximately 3. Interdisciplinary support includes physical therapy, interpretation services, phlebotomy, social services, lift teams, pharmacy, and transportation. There are expectations for case managers to float between various locations within the state as needed.
