Job Title: Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 8 Hours (08:00 - 16:30) Experience: Minimum 2 years of acute medical care manager experience License: CA state license required; pending license accepted but must be licensed upon start Certifications: None specified Must-Have: Experience with care coordination and discharge planning; ability to organize and participate in family meetings Description: Start date: ASAP Floating expectation to different facilities as needed. Patient ratio is 1:25. Experience in acute medical care coordination is required. First-timers will not be considered; travel experience is necessary. Weekend rotation is required. Competency in utilizing Epic for charting is a must. Experience should include assessing patients and managing discharge plans independently. Knowledge of disability case management, CMS guidelines, and compliance with HIPAA is essential. The candidate must be familiar with DME arrangements and transportation logistics. This role requires participation in treatment plan development and family meetings. Candidates should have knowledge of mental health issues and related treatment options. Ability to manage complex cases and work with interdisciplinary teams is expected. Must commit to working three of the five major holidays. Minimum of two supervisory references covering one year of employment within the last three years is required. All professionals should be open to floating between facilities within the state. Refusal to float may lead to termination of the assignment.
