Key Responsibilities of the Home Health RN Case Manager- Perform comprehensive assessments of high-risk members
- Collaborate with primary care providers to implement individualized care plans
- Address and reassess cost-efficient levels of care
- Coordinate non-medical support for treatment plan compliance
- Manage care transitions with healthcare team collaboration
- Maintain detailed clinical, functional, and fiscal records
- Identify opportunities for health promotion and illness prevention
- Act as a patient advocate, ensuring privacy and confidentiality
Requirements of the Home Health RN Case Manager- Associate or Bachelor's Degree in Nursing or related field
- Current, unrestricted State Registered Nurse license in Maryland
- CPR/BLS certification
- Valid driver's license and car insurance
- Wound care experience
- Proficiency in Point of Care/HomeCare HomeBase and OASIS
- Home health experience
- Case Management certification preferred
