Key Responsibilities of the RN - Case Management - Travel- Perform comprehensive assessment of high-risk members
- Collaborate with primary care providers to implement individualized care plans
- Address and reassess cost-efficient and appropriate levels of care
- Coordinate non-medical support to ensure compliance with treatment plans
- Coordinate care transitions and engage specialty resources as needed
- Keep detailed records of clinical, functional, and fiscal outcomes
- Act as patient advocate by protecting privacy and confidentiality issues
Requirements of the RN - Case Management - Travel- Associates or Bachelor's Degree in Nursing or related field
- Current, unrestricted CO RN license
- Ability to analyze complex medical information and make rational decisions
- Case Management certification
