This role involves working closely with high-risk members to ensure personalized and comprehensive care plans are in place, collaborating with a multi-disciplinary team in a dynamic healthcare environment.

Key Responsibilities of the RN - Case Management- Perform comprehensive assessment of high-risk members
- Collaborate with primary care providers to implement individualized care plans
- Reassess cost-efficient, appropriate levels of care
- Coordinate non-medical support to ensure compliance with treatment plans
- Coordinate care transitions with physicians, social workers, and claims professionals
- Engage specialty resources as needed
- Keep detailed records of clinical and fiscal outcomes
- Identify opportunities for health promotion and illness prevention
- Act as a patient advocate, protecting privacy and confidentiality
Requirements of the RN - Case Management- Associates or Bachelor's Degree in Nursing or related field
- Current, unrestricted Arkansas and Compact RN license
- Case Management certification
- Proficient with HCHB and Home Health Oasis
- 1 year of Home Health experience
- Ability to analyze complex medical information and make rational decisions
