Key Responsibilities of the RN - Case Management - Travel- Perform comprehensive assessment of high-risk members
- Collaborate with primary care providers to ensure individualized care plans
- Reassess cost-efficient and appropriate levels of care
- Implement non-medical support systems for treatment compliance
- Coordinate care transitions and engage specialty resources as needed
- Maintain detailed records of clinical, functional, and fiscal outcomes
- Identify opportunities for health promotion and illness prevention
- Act as patient advocate, protecting privacy and confidentiality
Requirements of the RN - Case Management - Travel- Associates or Bachelor's Degree in Nursing or related field
- Current, unrestricted state Registered Nurse license (can accept compact license)
- Case Management certification
- Home Health care experience
- Proficiency in HCHB and OASIS
- Med surg/critical care skills preferred
