Key Responsibilities of the RN - Case Management - Travel- Perform comprehensive assessment of high risk members
- Collaborate with primary care providers to implement individualized care plans
- Identify and reassess cost-efficient levels of care
- Ensure non-medical support for compliance with treatment plans
- Coordinate care transitions as needed
- Engage specialty resources for optimal resolutions
- Maintain detailed records of clinical, functional, and fiscal outcomes
- Identify opportunities for health promotion and illness prevention
- Act as patient advocate by protecting privacy and confidentiality
Requirements of the RN - Case Management - Travel- MS License (compact license accepted)
- Home Health care experience, HCHB, OASIS skills
- Associates or Bachelor's Degree in Nursing or related field
- Current, unrestricted state Registered Nurse license
- Ability to analyze complex medical information
- Case Management certification
