Key Responsibilities of the RN - Case Management- Perform comprehensive assessment of high-risk members.
- Collaborate with primary care providers to implement individualized care plans.
- Address and reassess cost-efficient and appropriate levels of care.
- Ensure non-medical support for treatment plan compliance.
- Coordinate with healthcare professionals in patient transitions.
- Engage specialty resources as needed for optimal outcomes.
- Maintain detailed records of management outcomes.
- Identify health promotion and illness prevention opportunities.
- Act as a patient advocate by protecting privacy and confidentiality.
Requirements of the RN - Case Management- Associates or Bachelor Degree in Nursing or related field.
- Current, unrestricted state Registered Nurse license (Florida or compact license accepted).
- Experience in analyzing complex medical information and making decisions.
- Competence in Home Care Home Base/Pointcare and OASIS collection.
